After crossing the bridge from Saria, enter the cave below you, and you'll appear by three caves. Enter the cave on the right, then go to the right through the cave until you emerge outside. Enter the cave on the right again, then go to the right through the cave until you emerge outside. Repeat this process another three times, then enter the cave below you. Go all the way to the left through the cave until you emerge outside.  

Enter the cave below you, then go all the way to the right through the cave until you emerge outside. Enter the cave below you, then go all the way to the left through the cave until you emerge outside. Outside, go to the left to the black hole in the ground. Walk over the hole to fall inside the cave. In the cave, go to the right to the elevator and take it down, then go all the way to the right to reach the Hammer.
