From the Pyramid in the Dark World, go right to the next screen, then go up to a river and follow the one-way path to a house (the Witch's house in the Light World). Go right past the house and continue along the one-way path to a group of rocks. Dash into the rocks to break them and go up the path on the right. Walk up to a river, then go left to a sign. Go left from the sign and pick up the skull. Go up and left a few steps and throw the skull into the circle of stones. A creature will appear and give Link the Quake Medallion.
